Polymyositis/Dermatomyositis

Polymyositis (PM) and dermatomyositis (DM) are rare inflammatory myopathies, autoimmune conditions that cause muscle inflammation and weakness, primarily affecting the proximal muscles like those in the shoulders, hips, and thighs. PM mainly involves muscle symptoms, while DM includes distinctive skin rashes in addition to muscle issues. These disorders can occur at any age but are more common in adults between 30 and 60, with DM also affecting children. Symptoms include gradual muscle weakness, fatigue, difficulty swallowing or breathing (in severe cases), and for DM, skin changes such as a violet-colored rash around the eyes, red bumps on knuckles, or a shawl-like rash on the upper body. Diagnosis involves blood tests to check elevated muscle enzymes, electromyography (EMG), muscle or skin biopsy, and imaging like MRI. While there is no cure, early treatment can improve outcomes with corticosteroids, immunosuppressants, biologic therapies, intravenous immunoglobulin (IVIG), and physical therapy.
